These high-risk patients often face challenges such as inadequate care coordination, poor transitions, suboptimal screening and management of social and behavioral drivers of care, deficient caregiver support, and fragmented disease management.
Vytal Care+ uses a longitudinal care coordination approach to support medically chronically complex patients through proactive care coordination, chronic condition education, and end-of-life support, while keeping PCPs at the center of the plan of care.
