Extend your care team with a clinical pharmacist, at no cost to your practice
Practices that partner with Vytalize Health gain access to Vytal Care™, a support program that pairs your patients with licensed clinical pharmacists between office visits. Vytal Care™ pharmacists reinforce the care plan you’ve set, identify medication-related opportunities, and route every recommendation back to you for review and final decision. You remain the clinical decision-maker; we extend your reach.
Vytalize’s pharmacy services are currently live in seven states: New York, Vermont, New Hampshire, Florida, Texas, Ohio, and New Jersey, with new markets added every month.
What your patients get
- A complete medication review. Prescriptions, over-the-counter products, vitamins, and supplements are assessed for effectiveness, interactions, duplication, and affordability. Patients also receive options to help lower their out-of-pocket costs.
- Support between office visits. Pharmacists maintain regular touchpoints with patients to reinforce your care plan through condition-specific support, including glycemic and blood pressure control, guideline-directed therapy support, adherence counseling and barrier resolution, injection and inhaler technique coaching, lifestyle support, and routine monitoring of glucose, blood pressure, daily weights, and symptoms.
- A health coach and a licensed clinical social worker (LCSW), as needed. Our health coaches and LCSWs help patients address behavior change, transportation, food, and in-home barriers that derail otherwise sound care plans.
What the evidence shows about pharmacists supporting PCPs
- Fewer admissions. In a published program evaluation from a primary care clinic operating as a PCMH within an ACO, embedding a clinical pharmacist was associated with roughly a 50% reduction in heart failure hospitalizations over the first 10 months, from 63 admissions to 30 among 111 patients followed. In a later 18-month analysis, 3 of 153 high-risk patients, or 2%, had a 30-day heart failure readmission. A separate 2024 analysis from a Michigan clinically integrated network found total admissions declined 16%, from 628 in the six months before patients’ first comprehensive medication management encounter to 526 in the six months after. (Schumacher et al., JMCP 2018; Earby et al., JACCP 2024)
- Lower total cost of care. In a Medicare ACO, a pharmacist-driven telehealth comprehensive medication management service for clinically complex patients reduced total medical spending by $2,331 per patient over six months, or approximately $388 per member per month, compared with controls, with an estimated return of 3.6:1 to 5.2:1. The Michigan network analysis showed a similar signal: among 1,199 patients, median total cost of care fell by $1,427 per patient in the six months after the first comprehensive medication management encounter, with significant reductions in both medical and prescription costs. (Urick et al., JMCP 2023; Earby et al., JACCP 2024)
- Why this matters for your practice. The patients in the Michigan analysis had a median of three chronic conditions, were taking six or more chronic medications, and were in the top 30% of spending. If that describes patients on your panel, they are precisely the patients this program is designed to support.
What partnership looks like
- Reinforce enrollment. Vytal Care™ identifies eligible patients and handles outreach. When those patients are in front of you, encourage them to participate. A recommendation from their trusted care team can make all the difference. Your EMR may even support the ability to one-click refer your patients into the program.
- Partner with the pharmacist on care plan goals and targets.
- Close the communication loop. Pharmacists communicate follow-up needs electronically through a workflow tailored to your practice’s EMR, and a dedicated Regional Medical Officer and Practice Transformation Manager guides your team through setup and ongoing support.
