Starting January 1, 2027, primary care practices participating in Medicare Accountable Care Organizations (ACO) will have two CMS accountable care models to choose from: the long-standing Medicare Shared Savings Program (MSSP) and the new Long-term Enhanced ACO Design (LEAD) Model, which replaces the REACH ACO program, ending in 2026.
Both reward practices for keeping patients healthy and lowering Medicare spending. But the rules, tools available, and risk profiles are meaningfully different, and the right choice depends on where your practice is on its value-based care journey.

What’s New in LEAD, and What It Means for Your Practice
Stronger support for ACOs serving complex and high-needs patients and those with historically higher spending: LEAD includes more stable benchmarking and risk adjustment for high needs and dually eligible beneficiaries, plus expanded alignment pathways and waivers tailored to complex populations. LEAD also provides incentives for ACOs with historically higher spending compared to their regional average to enter and remain viable.
Future Features: The 10-year time horizon before benchmark rebasing, a transition to AI-based risk adjustment in 2029, and changes in the methodology for trending benchmark growth after year 5, are long term features worth monitoring, but less material to up-front placement.
