As we progress further into 2025, I want to take a moment to reflect on what we’ve accomplished together and look ahead to what’s next. At Vytalize Health, our mission has always been to provide innovative and personalized healthcare solutions for providers and their patients, and last year we made strides that truly embody our commitment.

Recognized by Our Peers and Industry Leaders
We were honored to receive recognition from some of the most trusted names in our industry. Most notably, Vytalize earned the coveted No. 1 spot on the Inc. 5000 for our sustained innovation and growth. Deloitte also recognized Vytalize in their Technology Fast 500 list as the highest-placing private company, affirming our position as a leader in healthcare transformation. These accolades wouldn’t have been possible without the dedication of our team and the trust of the communities we serve.
Leading the Conversation Around Value-Based Care
Last year, we were proud to lead pivotal discussions shaping the future of value-based care (VBC). SVP of Global Accounting, Jessica Wijesekera, represented Vytalize at NetSuite’s Suiteworld, sharing insights that pushed the VBC conversation forward.
Additionally, Chief Marketing Officer Matt Buder Shapiro discussed the importance of transparency and trust within the healthcare system with VividFront for their “Marketing Moves” podcast. Matt’s appearance highlighted the key strategies driving Vytalize Health’s remarkable growth and deepening the bond between patients and providers.
Growth in IPA Efforts
Our independent practice association (IPA) model saw significant growth over the past year. We expanded our network, empowering more physicians with the tools and resources they need to deliver better patient outcomes. In October alone, IPANY welcomed 240 new members! This success underscores our commitment to fostering autonomy while improving care delivery.

Numbers tell a compelling story, and our performance metrics for PY23 are no exception. The average REACH participant earned $129,000 in advanced shared savings. These achievements reflect our focus on delivering value and making healthcare accessible and effective for all stakeholders.
Introducing New Developments
Innovation continues to be at the heart of everything we do. Last year, we launched a clinical decision engine that enhances how providers receive evidence-based recommendations. Additionally, our revamped transitions of care initiative reduced readmissions. These efforts represent the beginning of what’s to come as we continue investing in meaningful solutions.
Connections and Collaborations
Last year was also notable for the partnerships we formed and strengthened. We teamed up with WellSky to enhance value-based outcomes for patients by seamlessly sharing critical information across care settings, ensuring discharges to high-performing post-acute facilities. This collaboration is helping us create scalable, impactful solutions for patients and providers alike.
Network Updates
Finally, we’re thrilled to report significant progress in expanding and improving our network. By the end of 2024, we increased our network to include over 11,000 specialist providers. This growth reflects our ongoing dedication to creating a robust, inclusive network that meets evolving healthcare needs.
