This new chapter couldn’t come at a more exciting time, as we continue to roll out additional multi-pronged initiatives that address physician practices’ major pain points.
Today, Vytalize supports a curated and rapidly expanding network of 1,000 physician practices, 3,000 primary care providers, and 7,000 specialists. We provide end-to-end support in VBC through an AI-enabled clinical decision support platform, data analytics, financial incentives, practice transformation support, along with direct clinical support to help the nation’s leading practices perform even better in VBC.
We’re excited to share that the data shows conclusively that our approach is working. Vytalize’s practices consistently improve over time. Mature Vytalize cohorts generate over 8% savings—approximately $1,400 per patient annually by their 3rd year. Importantly, these savings reflect actual reductions in healthcare utilization, and are independently verified and isolated from risk adjustment and other revenue-optimization factors. This validation comes from seven years of data encompassing 600,000 patient-years and rigorous third-party actuarial analyses, confirming true healthcare cost reductions beyond improvements in clinical documentation alone.
Using proprietary clinical decision support tools such as Vytal Insights™ and peer-to-peer guidance tied to financial incentives through our Vytal Signs™ program, Vytalize provides partnered practices with advanced enterprise-level tools to succeed and generate shared savings. Going beyond primary care, our Vytal Network encompasses a curated network of specialists, post-acute providers, and hospitals that collaborate with PCPs and each other to deliver superior and affordable care.

Inpatient costs remained significantly lower, from $3,271 at Year 0 to $3,764 by Year 4+, versus regional averages rising from $3,403 to $4,213.
Skilled Nursing Facility (SNF) costs decreased notably, reaching $1,374 by Year 4+, well below the regional average of $1,663.
Outpatient services costs consistently stayed below regional benchmarks, starting at $2,605 and reaching $2,814 at Year 4+, compared to $3,547 regionally.
Home Health and Hospice services demonstrated substantial improvement, declining from $901 initially to $734, significantly outperforming the regional benchmark of $946.
