We knew a better care delivery model was needed for Medicare patients and, in 2014, set out to solve the problem. We built our first practice in New York City. Our practice took care of the whole person, was available 24/7 and on-demand, and met patients wherever needed, including virtually and at-home.
After expanding our footprint to New Jersey in 2016, we determined that there was enough waste in the healthcare system, and that a truly impactful business model would need to have capital efficiency as a core tenet. With that core belief in mind, we decided to deploy our data-driven, Medicare-focused care delivery model in partnership with other practices rather than building new practices from scratch.
In 2018, we wrapped our successful clinical care model with an enhanced business model that allowed us and our physician partners to share in cost savings that we generated for Medicare.
Today, our care delivery model transforms the healthcare experience for over two hundred and sixty thousand Medicare beneficiaries across 36 states by helping them take control of their chronic conditions in collaboration with their doctors.
Our multidisciplinary clinical team serves as an extension of each physician to design custom-tailored care plans for each of their Medicare patients that are proven to prevent & reduce hospitalizations and improve quality of life.
