Determined to transform outdated care models for Medicare patients, we launched our first practice in New York City in 2014. We put the focus on whole-person care, offering 24/7 support and visiting patients wherever they needed us—virtually or even at home. And we saw the results in patient outcomes.
By 2016, we expanded to New Jersey and realized the true potential to transform healthcare lay in partnerships. We needed a scalable, data-driven approach that enabled practices to tackle inefficiencies in the healthcare system head-on.
In 2018, we created a powerful partnership platform. This approach brought Medicare-focused care innovations to participating practices and allowed us to share the cost savings we generated for Medicare, strengthening outcomes for all involved.
Today, Vytalize Health reshapes healthcare experiences for over 350,000 patients across 30 states. Supporting doctors with our multidisciplinary clinical teams, we custom-build care plans to reduce hospitalizations and improve patients’ quality of life.
